　乡村卫生服务一体化管理工作方案

　为全面提升乡村卫生服务一体化管理水平，确保巩固完善基层医改政策的落实，根据无为县卫生局《关于印发无为县乡村卫生服务一体化管理细则（试用）的通知》（卫办【2011】339号）文件精神，结合我镇乡村卫生服务一体化管理开展的实际情况，制定本方案：

　一、工作目的：

　通过实行乡村卫生服务一体化管理，进一步加强乡镇卫生院对村卫生室的业务指导和管理，规范服务行为，为群众提供方便、价廉、安全、有效的基本医疗和公共卫生服务。

　二、工作内容：

　全镇村卫生室实行乡村卫生服务一体化管理，实现机构、人员、业务、药品、财务、绩效考核“六统一”管理。

　（一）统一机构管理

　1、坚持一个行政村设立一个村卫生室的原则，并统一机构名称、统一标牌标识。。

　2.坚决取缔无资质人员非法行医或流动行医；定期、不定期实行明察暗访。

　（二）统一人员管理

　我院对村级卫生人员采取聘用制医生方可由院择优聘任，统一调配使用乡村医生发放。1、

　（三）统一业务管理

　1、加强业务督导:每月初由院长或防保站长率相关工作人员，对村卫生室的基本药物制度执行情况、药品零差率实施情况，基本公共卫生均等化服务项目工作实时完成情况，村级门诊统筹工作开展情况，开展针对性督查，对存在的问题提出限期整改措施。

　2、定期业务培训：每月下旬召开乡村医生例会，注重解决在乡村卫生服务一体化管理过程中存在的普遍问题、突出问题。加强对建立居民健康档案的服务质量、0-6岁儿童健康管理、老年人健康管理、高血压患者健康管理、2型糖尿病健康管理、重性精神病管理、传染病及突发公共卫生事件报告、医德医风、常用医疗技术操作、外伤处理、现场急救、突发医疗事件应急处理等业务进行培训，指导和规范。同时要求村卫生室人员在日常诊疗工作中做到看病有登记用药有处方收费有据公共卫生服务有　、　、做到收费有单据、账目有记录、支出有凭证。2、、

　在卫生局统一领导下，成立开城镇中心卫生院乡村卫生服务一体化管理工作领导小组，具体人员如下：

　我院对所属的村卫生室实行绩效考核，对16个村卫生室实行分片责任包干，由相关责任人对村卫生室人员进行帮助，监督，指导完成乡村卫生一体化服务工作。并对责任人制定包干责任村卫生室达标标准和责任包干人员奖惩办法，使工作落到实处，确保达到责任包干效果，促使我镇乡村卫生服务一体化管理工作跃上一个新台阶。
